North Bay Aquatics
Entry Form

How to enter an away meet

  • Please read the text on the first page of the meet sheet so you know what is going on. Most meets like this have the same info, so after you read it once , you can kind of skim to the important info from now on.
  • Figure out what events you would like to swim. Although most kids only want to swim their favorite, I like to see kids swim lots of events. You get better through experience and you avoid the hanging out waiting forever to participate. Obviously there are some events that are not appropriate for some beginning /young swimmers. Certainly you can ask Mike or myself if you have questions about this.
  • Go to the second page and find the event numbers of the events you want to swim in.
  • Fill out the consolidated entry form. Further instructions on how to complete the form are below, along with a sample form.
  • You are responsible for completing your entries and mailing them with payment. Be sure to mail your entry early. Late entries are not permitted and meets often fill up before the due date. DO NOT GIVE THE ENTRIES TO KEN.
  • Club Code--we are the NBA
  • Club Name--we are North Bay Aquatics, NOT TPC
  • LCS--- we are PC (stands for Pacific Swimming ,our governing body)
  • USS REG NUMBER--use your 6 digit birth date (ie June 12 1992 would be 061292), then the first 3 letters of your first name, your middle initial (if you don't have a middle name, use a * in that slot), and the first 4 letters of your last name.
  • Enter your event #, the distance and stroke and the best time for the event in the proper slots. If you don't have a time, enter NT, or ask Mike or myself for an estimated time.
  • Figure out your fees. Usually there is a $2 or $3 per entry fee and a splash fee around $5 to help rent the pool that we will use.
  • Fill out your address and phone number. This is important as if there is a problem with your entries, they can call up before the day of the meet to fix that problem.
  • Look on the meets sheet on who to make the check out to and where to send the entries. DO NOT GIVE THE ENTRIES TO KEN.
  • If you are an official for stroke and turn or a marshal, complete the 'my parent is an official' section to offer to work the meet. If you are not an official, please help out by timing but you do not complete this section. Not all meet entry forms have this section.
